Covid-19 has been declared a worldwide pandemic, by the World Health Organization. Covid-19 has been declared as contagious. By signing the agreement, I acknowledge and agree to the following….

1.     Covid-19 is contagious

2.     No one at Parkview Christian Church can guarantee that your child will not pick it up from some one else during an event. (However, taking temperatures upon arrival, and providing sanitizer, and other safety precautions will be encouraged while attending)

3.     I recognize that by allowing my child to participate in Youth Group, or any other Student ministry function/event at Parkview Christian Church, there is always a chance my child can pick up Covid-19 or any illness/virus from others, the same as they could at school, the grocery store, restaurants, etc.

4.     I voluntarily and unconditionally assume all of the foregoing risks, and all risks associated with Covid-19 and all other viruses, and any and all risks associated with participation in Parkview Christian Church Events

5.     On my behalf, and behalf of my children, I hereby release, covenant not to sue, discharge, or hold harmless, Parkview Christian Church staff, leaders, members, volunteers, supporting ministries, etc., should Covid-19 or any other virus be transmitted to my child or any member of our family,  during or from a youth event.

6.     I fully understand everything in this agreement.
